During 2013, the State of Delaware embarked upon a major health planning initiative, called the State Innovation Model (SIM) project with the goal of improving the health of Delawareans, improving the patient experience of care, and reducing health care costs – the Triple Aim. This endeavor, funded under a grant from the Centers for Medicare and Medicaid Innovation (CMMI), has involved the participation of many health care stakeholders in the state, including representatives of the Medical Society in each of the project’s six work groups that discussed specific transformation topics: 1) Delivery System Transformation; 2) New Payment Models; 3) Data and Analytics; 4) Population Health; 5) Workforce; and 6) Policy.
